PLEASE LIST ANY INDIVIDUALS WHO ARE AUTHORIZED TO PICK UP YOUR CHILD FROM ART CAMP. YOUR CHILD WILL NOT BE RELEASED TO ANYONE WHO IS NOT ON THIS LIST, UNLESS REQUESTED IN WRITING BY PARENT.

Please list any allergies your child has (food/medication/etc.) and any other medical conditions that may impact his/her participation. *

RELEASE OF LIABILITY: I hereby hold Amy Waldrop and Clifton Art Studio harmless from any liability relating to any injuries resulting from an accident, illness (including COVID-19) or allergic reaction my child may have while under the care of Amy Waldrop/Clifton Art. I understand that by registering my child, I agree that I will not send him/her to camp with a fever or any symptoms of or exposure to COVID-19. I understand that I will be notified in the event that any child reports COVID symptoms or exposure. I understand that in the event of any emergency, Amy Waldrop will attempt to contact me immediately. If I am unreachable, she will contact my emergency contact. I authorize Amy Waldrop/Clifton Art to use their discretion when seeking emergency medical assistance for my child. *

2025 ART CAMP REGISTRATION

New and returning students are asked to complete this registration form and will be added to the class on a first-come, first-served basis, with returning students given priority registration. In the event that your desired session is full, you will automatically be placed on a waiting list (no deposits or payment is required until your acceptance into the class) and you will be notified via email with a listing of other open sessions. Depending on demand, additional sessions may become available. Please complete ALL areas marked with an asterisk, or form will not submit.

REFERRAL PROGRAM: If you are the first to refer a friend that has never been to the studio or taken classes or Art Camp with Amy and they sign up for Art Camp, you will receive a $20 discount for the week! Please submit your referal below, or email amy at info@cliftonartstudio with your list of referals. In the event that two people refer the same new student, the registration with the referal that comes in first will receive the credit.

NOTE: Once your registration form is submitted, you will be sent an email with confirmation and payment instructions within 72 hours. A 50% non-refundable deposit is due with each registration to secure your child's space in the camp. Please register my child for the following Art Camp Session. Please submit one form for each requested session. The cost for each week-long, half-day session is $325.00 for your first session and $700.00 for each full day weekly session. All supplies are included. Students are asked to wear paint-friendly clothing and to bring a snack. Complimentary filtered water is available for all students. Please note: Private group sessions are available for sibilings and friends groups. Scheduled upon availability and request (4 student minimum). Additional weeks may be added. *

PARENT PERMISSION 2: I hereby authorize Amy Waldrop and Clifton Art Studio to post images of my child and/or their artwork on social media for advertising purposes. Artwork will be labeled with the child's first name only. Your child will retain all rights and privileges to her images. Children's artwork will not be made available for sale. *
